ScienceDaily (July 27, 2012) ? Hypothermia in trauma victims is a serious complication and is associated with an increased risk of dying. A new study published in BioMed Central's open access journal Critical Care has found that the key risk factor was severity of injury. However, environmental conditions and medical care, such as the temperature of the ambulance or temperature of any fluids administered intravenously, also increased risk.
A multicentre study, carried out by the emergency medical services of eight hospitals across France, looked at the injuries, care and outcomes for all adult trauma victims, over a three year period, who received pre-hospital care and were transported to hospital in an ambulance. Body temperature was continuously measured using a infrared tympanic thermometer and hypothermia was defined as below 35C.
The study found that 14% of patients had hypothermia on arrival at the hospital. Dr Fr?d?ric Lapostolle from SAMU 93 -- Unit? recherche-enseignement-qualit?, H?pital Avicenne, who led this study, explained, "As expected the severity of hypothermia was linked to the severity of injury. Blood loss and spine or head injury impair body temperature regulation and in our study we found that head injury, and intubation to aid breathing, were independently associated with hypothermia."
While external air and ground temperatures seemed to have little effect on risk of hypothermia, the study found that infusion fluid temperature and the temperature inside the ambulances were both significant risk factors.
Dr Lapostolle continued, "The temperature of infused fluid for 75% of our patients was below 21C and usually at ambient air temperature. We suggest that, to reduce the incidence of hypothermia, the temperature of infusion fluids need to be controlled, and that a small a volume as possible is used. Temperature of infusion fluids can be easily and rapidly measured in pre-hospital settings. We also recommend that ambulances should be heated and that as much as possible the patient should remain clothed, because attempting to warm the patients did not compensate for the effect of them being undressed even if it can make examination more difficult."

ScienceDaily (July 31, 2012) ? Roughly 1.8 million Americans have celiac disease, but around 1.4 million of them are unaware that they have it, a Mayo Clinic-led analysis of the condition's prevalence has found. Meanwhile, 1.6 million people in the United States are on a gluten-free diet even though they haven't been diagnosed with celiac disease, according to the study published July 31 in the American Journal of Gastroenterology.
Researchers have estimated the rate of diagnosed and undiagnosed celiac disease at similar levels prior to this study, but this is the most definitive study on the issue. "This provides proof that this disease is common in the United States," says co-author Joseph Murray, M.D., a Mayo Clinic gastroenterologist. "If you detect one person for every five or six (who have it), we aren't doing a very good job detecting celiac disease."
Celiac disease is a digestive disorder brought on when genetically susceptible people eat wheat, rye and barley. A gluten-free diet, which excludes the protein gluten, is used to treat celiac disease. Roughly 80 percent of the people on a gluten-free diet do so without a diagnosis of celiac disease.
"There are a lot of people on a gluten-free diet, and it's not clear what the medical need for that is," Dr. Murray says. "It is important if someone thinks they might have celiac disease that they be tested first before they go on the diet."
To determine its prevalence, researchers combined blood tests confirming celiac disease with interviews from a Centers for Disease Control and Prevention (CDC) nationwide population sample survey called National Health and Nutrition Examination Survey. The survey, designed to assess the health and nutrition of U.S. adults and children, is unique in that it combines interviews and physical examinations.
Researchers found that celiac disease is much more common in Caucasians.
"In fact, virtually all the individuals we found were non-Hispanic Caucasians," says co-author Alberto Rubio-Tapia, M.D., a Mayo Clinic gastroenterologist. But previous research in Mexico has shown that celiac disease could be just as common as it is in the U.S.
"So that is something we don't fully understand," Dr. Rubio-Tapia says. The study found the rate of celiac disease in the U.S. is similar to that found in several European countries.
The research was funded in part by the National Institutes of Health and the CDC. Study authors include James Everhart, M.D., from the National Institute of Diabetes and Digestive and Kidney Disease; Jonas Ludvigsson, M.D., Ph.D., from Orebro University Hospital and the Karolinska Institutet; and Tricia Brantner from Mayo Clinic.

One moment I was lying in bed, calmly reading a novel. Then the main character became pregnant and I snapped. I got up and searched through my jewelry box for the pointiest pin I could find. I pulled out a large, vintage yellow daisy pin. I grabbed the condoms from the night table and started poking holes in them. Poke, poke, poke. Jab, jab, jab. The pin was two large and left big, gaping, noticeable holes. Just like the ones I felt inside of me.
Looking at those holes in the silver wrapper was a big wake up call for me. I hid the condoms under tissues in the bathroom garbage can, sat back on the bed and sobbed. I?d been hiding my feelings for so long. I hadn?t allowed myself time to mourn or grieve. The pain couldn?t be held back any longer and came out in a big ball of crazy condom poking.
I had experienced a miscarriage several months before. The pregnancy wasn?t planned. In fact, babies weren?t in the plan at all. My husband made it clear from the beginning that he didn?t want children. I told him that I was willing to sacrifice babies for him. I actually thought he?d change his mind. He didn?t and I struggled with letting go of my strong desire to have a child.
We were in a really stressful place about six years into our marriage. We were trying to sell a house in a market where no one was biting, after feeling forced to vacate it due to harassments and threats from the people across the street. We were living in an empty home owned by my mother-in-law until our house sold and provided us with the funds to buy again. We weren?t happy living there and the situation created all kinds of family drama. Finances were tight. My husband was a full time student. His father was extremely ill. We were stressed to the max. I forgot to take my birth control pill for three days.
I was sure the exhaustion, headaches and nausea were from the stress. I thought stress was also what was delaying my period and that my breasts were so incredibly sore because of PMS. I assumed I was having a bad reaction to my toothpaste when I threw up several mornings in a row. Being pregnant didn?t even cross my mind. Sex was scarce during that time, so I didn?t think much of it. Plus, I?d missed doses here and there in the past without problem.
Then I woke up in the night and a pool of blood hit the floor the moment I stood up. Pregnancy still didn?t enter my head. I thought my period must be extra strong because it was late. I called my gynecologist the next day when the heavy bleeding continued. The doctor called it a ?missed pregnancy?.
I was numb and in shock. I stayed in bed crying and eating chocolate peanut butter ice cream for a few days, but I didn?t fully deal with my feelings. I shoved them down. I went back to work. I pretended I was okay. I told myself I was fine.
I wasn?t fine. Women who are handling things ?fine? don?t poke holes in condoms. I was a mess.
My husband was sad when he learned of the miscarriage, but it was only because he knew I was hurting. He was relieved there would be no baby and terrified pregnancy would occur again. I finally realized that he wasn?t going to change his mind. No matter how much he adored me, he did not want a baby.
Would I have actually gone through with using the condoms if the holes weren?t so big and noticeable? I like to think I wouldn?t, but I don?t know for sure. I?m glad the holes were so glaring. It forced me to stop what I was doing and to acknowledge my grief and pain.
I opened up to my husband about all the feelings swirling around inside of me. I wasn?t just mourning the loss of my pregnancy, but the hope of any future pregnancies. I felt so ripped off, like the universe was playing a cruel joke on me by allowing me to get pregnant, but then miscarry before even getting the chance to be happy or excited about the prospect of motherhood.
We talked and talked. The conversation kept coming up again and again for months. I had a lot to process. Through these talks two big points became clear. My husband wasn?t totally opposed to being a father, he just didn?t want a baby. I just wanted to be a mother and how it happened actually wasn?t important to me.
We?d thrown around the possibility of older child adoption for years, but never seriously talked about it prior to this. We started to really consider it. We made it a tentative ?some day? plan. I dove into research. I was shocked when I told my husband about upcoming classes to get licensed to adopt from the foster care system and he said, ?Let?s sign up.?
A year after we officially started the process, our daughter moved in with us. She was nine years old and had been in foster care for five years. She had suffered abuse, neglect, poverty, homelessness, abandonment, instability and many other things children should not have to face. We finalized the adoption six months later.
Parenting a traumatized child is challenging, but it is also so very rewarding. Our daughter has made huge progress since coming home to us. She?s learning to control her anger, work through her feelings and trust us. I felt a pull to her from the moment I saw a photo of her sweet face. She is my daughter. My baby. I was made to be her mother. My husband is an amazing father. Nothing brings me more joy than watching the two of them laugh together. She has healed me. She has completed me. The holes in my heart were waiting for her to fill them.

Dr. Peter Geller visited London in 1996 on a hunch. Lucky for both him and his research, his gut feeling turned out to be correct.
?While most people go to London to see the sights and watch the theatre, I sat in an archive and watched a film,? he says with a laugh.
That film was The Romance of the Far Fur Country, a silent documentary shot by filmmakers hired by the Hudson?s Bay Company (HBC). The silent documentary was filmed in 1919, premiered to a Winnipeg audience in 1920 in honour of the HBC?s 250th anniversary, but then fell out of the public eye.
However, Geller, the Vice Provost and Associate Vice-President Academic at University of the Fraser Valley, suspected the reels might be housed in the National Film and Television Archives at the British Film Institute. Sure enough, upon his arrival in London, he was able to track down the film in the archives. The only problem? It was far from a finished film: the footage was completely out of order.
Geller?s research has long revolved around the visual history of the Canadian North, which this film fit into perfectly. After watching and studying the footage in London, he included a chapter about The Romance of the Far Fur Country in his 2004 book, Northern Exposures: Photographing and Filming the Canadian North, 1920-1945.
Spurred by the interest of Geller and filmmaker Kevin Nikkel, the footage was returned from Britain to the HBC Archives in Manitoba earlier this year. The footage was digitized prior to its return to Canada in the fall.
But their work didn?t end there. Now captivated by the film footage, Geller and Nikkel have been working together to piece the film back into some kind of order so it can once again be shown to a Canadian audience.
?There is eight hours of raw footage, which the original filmmakers edited into a two-hour-long documentary,? Geller notes. ?The footage we found in the London archive isn?t the final film ? it?s a jumble of images and out of order captions. There?s a lot of work to be done to piece it back together.?
The team has now assembled the footage into a 30 minute film, which is being shown across Canada in the coming months. It?s already played in Winnipeg, Ottawa, Montreal, Edmonton and several communities in northern Alberta, including Fort Chipewyan, and is coming to Vancouver on July 31. After that, it will travel to Victoria and onto Alert Bay.
Geller says he hopes to help bring the film to as many communities as possible ? especially the communities the original film team visited in 1919.
?We?re basically retracing the steps of the film. We?re making an effort to return the footage to the communities, since it was never screened in the communities it was shot,? Geller explains. ?It?s this huge, cross-Canada odyssey, and we want to connect that back to its roots.?
The original film crew started in Montreal, travelled up the St. Lawrence to Labrador, up to what is now Nunavut, into northern Ontario, and then over to northern Alberta, and finally over and up the sunshine coast to Alert Bay. Geller and Nikkel hope to follow at least some of that journey.
?It?s a fascinating film that helped to draw Canada together,? Geller says. ?The HBC hoped to show how they, as a company, had helped bind Canada together ? connecting Inuit and other northern Aboriginal peoples to traders and city dwellers like the original Winnipeg audience. For the Hudson?s Bay Company, it was a celebration of its history and showed the audience the variety of culture across Canada. And as a document of its time, it is important to recognize how the film is a part of the history of colonization, and both presented and shaped stereotypical views of Aboriginal people.?
The film will be showing at Vancity Theatre in Vancouver on Tuesday, July 31 at 7 pm. Geller and Nikkel will both be attending, and will offer a short Q & A period after the screening.
A spokesperson for Dr. Geller said, "Dr. Geller hopes to screen in as many places as possible, and we might see screenings in the Fraser Valley in the near future."

Rebels captured a government military base Monday on the outskirts of Aleppo, the hotly contested Syrian metropolis that has seen more than a week of bloody clashes.
The base had about 200 Syrian troops and appeared to be under attack by rebels from three sides overnight.
"The battle lasted around nine hours," said Fazad Abdel Nasr, a rebel commander working in the northern Aleppo suburbs. Nasr said six regime soldiers and four rebel fighters were killed.
The rebels also gained heavy equipment to supplement the lesser weapons they had been fighting with.
"We captured four tanks in good condition, and they are now in the hands of the Free Syrian Army. Two were destroyed," Nasr added.
Despite the boost for the rebels, the relentless battle for Syria's largest city raged on Monday.
Al Jazeera correspondent Omar Khashram was wounded during heavy fighting in a central Aleppo neighborhood. A cameraman and driver working with Khashram, who was being treated in a hospital in Turkey, told CNN that shrapnel from a shell penetrated gaps in the correspondent's flak jacket in Aleppo's Salahuddin neighborhood.
U.N. observers have reported a surge in violence in Aleppo, with helicopters, tanks and artillery being used, mission head Lt. Gen. Babacar Gaye said Monday.
Regime forces launched missiles and shelled from attack helicopters, opposition activists said.
Aleppo, Syria's commercial and cultural hub, has seen a mass exodus amid the violence. About 200,000 people in and around the city have fled shelling and heavy weapon fire in the past two days, Valerie Amos, the U.N.'s under-secretary-general for humanitarian affairs, said Sunday.
There were conflicting reports Monday on who controlled the major Aleppo neighborhood of Salahuddin, which rebels had claimed days earlier. Both opposition fighters and the regime said Monday they had taken over Salahuddin.
At least 17 people were killed across Syria on Monday, including eight in Aleppo, according to the opposition Local Coordination Committees of Syria.
With no end to the country's 16-month crisis in sight, French Foreign Minister Laurent Fabius said his country will take more action to try to stop the bloodshed.
"As France is taking over the presidency of the U.N. Security Council on August 1, we are going to ask -- before the end of the week -- for a meeting of the Security Council, probably at a ministerial level ... to try and stop the massacres and prepare for the political transition," Fabius told French RTL radio on Monday.
Also on Monday, the top diplomat at the Syrian Embassy in London resigned his post, the British Foreign & Commonwealth Office said. Charge d'Affaires Khaled al-Ayoubi told British officials that he was "no longer willing to represent a regime that has committed such violent and oppressive acts against its own people," the office said in a statement.
Earlier Monday, a Syrian brigadier general -- who is also a deputy police chief of Latakia -- defected to Turkey, the Turkish Foreign Ministry said. An additional 11 Syrian officers also defected overnight, the ministry said. The total number of Syrians who have fled to Turkey has reached 43,500.
Meanwhile, fighting words continued between Syrian President Bashar al-Assad's regime and opponents near and far.
Syrian Foreign Minister Walid Moallem gave ominous words Sunday about the battle for Aleppo, vowing that rebels would not gain control of the city.
"Since last week, (opposition fighters) planned for whatever they called the 'great Damascus battle,' but they have failed after one week," Moallem said, referring to a rebel offensive beaten back earlier this month. "That's why they moved to Aleppo, and I can assure you that they will fail."
He made his comments during a trip to Iran, one of his nation's few remaining allies.
But U.S. Defense Secretary Leon Panetta predicted Sunday that the Syrian regime's violent crackdown in Aleppo will prove "a nail in Assad's coffin" by turning even more people against al-Assad and his government.
Nabil Elaraby, the head of the Arab League, said Sunday that the league believes war crimes are being committed in the city.
At least 114 people were killed across Syria on Sunday, according to the LCC; 11 of the dead were from Aleppo.
Elsewhere, 41 people were killed in Damascus and its suburbs, including 18 people whose bodies were found charred on farms in Moadamiyeh, the LCC said.
Syrian TV reported that "terrorists" suffered heavy losses after clashes in three Aleppo neighborhoods.

A dozen riders were stranded for nearly two hours atop a newly built roller coaster in California before the ride was repaired and rolled the amusement park-goers safely to the ground.
The Superman Ultimate Flight roller coaster stopped at about 2:30 p.m. PT Sunday at the top of the ride, Six Flags Discovery Kingdom spokeswoman Nancy Chan told the Vallejo Times-Herald.
Local firefighters were called in and used a special crane and bucket to bring water to the stranded riders. The contraption could have been used to bring down the roller coaster riders if the coaster wasn't restarted, Vallejo Fire Department Battalion Chief Ray Jackson said.
But eventually, the workers got the cars unstuck and they rolled backward along the tracks to the ground.
There were no injuries reported.
It was unclear what caused the two-car train to stop.
"If (a ride) stops, it usually detects something and it stops for safety reasons," Chan said.
Chan told the Times-Herald that the ride will be closed for a thorough safety inspection, and it will reopen when the inspection is completed, at an undetermined time.
The ride, which opened June 30 at the Vallejo theme park, can travel up to 62 mph. It? holds two cars with six riders each. Parts of the ride include the cars being tilted upside down.
Luckily, the riders weren't stuck in that position.

TITLE : Alexandria Library - Oath of Allegiance in Virginia, 1862-1865
DESCRIPTION : The Oath of Allegiance, also called a Loyalty Oath, was a document signed by persons during and after the Civil War to pledge loyalty and allegiance to the Union. Initially intended for employees of the Federal government and military personnel, the oath soon took several different forms and eventually extended to the state level. Employment and business ownership were then dependent on signed oaths. The database is a list of approximately 1,200 persons, many of whom were residents of Alexandria, who signed the Oath of Allegiance.

I?ve been investing in precious metals, but before I invest anymore I want to get a feel for the possibility of a gold standard in the US. Ron Paul would fight for it, and Gingrich says he would too (although I think he?s bullshitting). How would that affect my investment?

CHICAGO (AP) ? Jesse Jackson Jr.'s transfer to the Mayo Clinic in Minnesota could indicate a complicating physical illness arose during the Illinois congressman's treatment for depression, several experts in psychiatric care said Saturday.
The Chicago Democrat has been on a secretive leave of absence for nearly seven weeks, during which his office has released only occasional snippets of information, including that he was undergoing treatment for a "mood disorder" at an undisclosed location.
On Friday, the Mayo Clinic distributed a statement from the congressman that said he had been transferred there for "extensive inpatient evaluation for depression and gastrointestinal issues." The clinic would not release more information Saturday.
John Anderson, of the Associates in Psychiatry and Psychology in southeastern Minnesota, said people receiving psychiatric care are often transferred to the Mayo Clinic when a physical illness develops because both can be treated there.
"Mayo does an excellent job in terms of combining those, so they can treat what's essentially a dual diagnosis," he said.
He noted specifically that Mayo Clinic has a highly rated gastrointestinal department as well as a free-standing inpatient psychiatric unit.
Jackson underwent a procedure in 2004 to help him lose weight that involves removing part of the stomach and rearranging the intestine. Phone messages left Saturday for Jackson's spokesman weren't immediately returned.
Friday's statement was the first to mention a gastrointestinal illness, and it was not known whether that was related to his depression or an entirely separate medical issue.
The statement did not disclose where the longtime Chicago congressman, the son of civil rights leader Jesse Jackson, had been staying previously.
He went on leave June 10, though his office didn't disclose it until weeks later and has been mum on details ever since. Initially, his office said Jackson was being treated for exhaustion. But his staff later said Jackson's condition was more serious and required inpatient medical treatment. They also said Jackson has been grappling with emotional issues.
Under mounting pressure from his colleagues and constituents, his office released a statement last month from his unnamed doctor saying Jackson was receiving intensive medical treatment at a residential treatment facility for "a mood disorder."
The timing of his medical leave has raised questions, in part because Jackson is facing an ethics investigation in the U.S. House connected to imprisoned former Illinois Gov. Rod Blagojevich.
The House Ethics Committee is investigating allegations that Jackson was involved in discussions about raising money for Blagojevich's campaign in exchange for the then-governor appointing him to President Barack Obama's vacated U.S. Senate seat. Blagojevich is serving a 14-year prison sentence for corruption.
Jackson was not charged and has repeatedly denied wrongdoing.
Jackson faces Republican and independent candidates in November, though he's widely expected to win re-election. He first won office in a 1995 special election and has easily won each race since. His district includes parts of Chicago and some suburbs.

CARSON CITY ? Some consumers in Nevada can soon expect to receive rebates from their health insurance companies.
A provision of the federal Affordable Care Act requires that health insurance companies spend a certain percent of a consumer?s premium dollars on medical care and quality improvement. This is called a medical loss ratio. Those companies that didn?t meet the medical loss ratio standard in 2011 will be required to provide rebates to their policyholders no later than August 1.
Rebates will be issued to 54,290 Nevadans.
While the amount of each rebate will vary and depends on the premiums paid by the consumer, the average rebate will be about $96 per person, equal to $5.75 dollars a month for the previous plan year. The average health insurance premium paid in 2011 was about $4,200 per person per year, or $350 per month.
?Our health insurance market is a competitive marketplace, with 40 active carriers. Premiums consumers pay are competitively priced, and that is evidenced by the relatively low amount of rebates, $4.5 million in rebates compared to $2 billion in total premiums,? said Insurance Commissioner Scott J. Kipper. ?The Nevada Division of Insurance continues to monitor the markets to ensure our carriers comply with Nevada?s premium pricing statutes. This ensures that our consumers get the best value for their premium dollars.?

Searchers on Saturday found the bodies of two U.S. mountaineers who apparently plunged 1,000 feet to their deaths on their way down from the summit of a glacier-capped Peruvian peak.
Gil Weiss, 29, and Ben Horne, 32, fell off a ridge after reaching the west summit of 20,584-foot?Palcaraju in the Cordillera Blanca range in mid-July, search coordinator Ted Alexander told The Associated Press.
Their bodies will be recovered Sunday, he said.

Both Weiss, of Queens, N.Y., and Horne, of Annandale, Va., were experienced climbers. Weiss was a repeat visitor to the Cordillera Blanca while this trip was Horne's first.

Both belong to the pullharder.org climbers' collective, and Horne wrote about the first, six-day leg of their trip on its blog, saying they had been buffeted by hurricane-force winds when the two reached the top of the 20,216-foot Ranrapalca.
After a rest in Huaraz, the two set out again July 11 for an excursion of seven to 10 days. Their families contacted Alexander after 13 days passed with no word from them.
Weiss's sister, Galit, said the two were not carrying a satellite phone.
